CMS has updated their Electronic Fund Transfer (EFT) Authorization Agreement forms needed to enroll or update Physicians information with Medicare. The new form is the CMS-588 and all previous unused paper copies and computer versions of the form should be discarded. “Form CMS-588 (4/10)” will be in the bottom left corner of the new form and it must be filed when you submit, update, or revalidate enrollment information. The new form requires less information about the contact person and that is the major change.

To make sure you have the latest forms available for CMS and other organizations, you should not keep copies of printed forms or saved computer files around the office. It is best to bookmark the organization's forms directly from their website to insure you have the most currently accepted form.
